Reset Password
Note
Passwords are encrypted for security before they are stored in the database. Once encrypted, they can not be decrypted, therefore, it is impossible to recover a forgotten password. Fill out the form below and your password will be reset to a new temporary password which will be sent to you via email and/or SMS depending on the preference you set in your profile.
The first time you log in with your new temporary password, you will be asked to change it to a password you choose.
If you do not have an email address

SMS (cell phone) number stored in your profile then you will not be able to reset your password here. You will have to contact an Administrator to reset it for you.
